Submitting your supporting photos and evidence for weight loss treatment

What we need and how to submit it

For all new Phlo Clinic weight loss patients, we need to verify your weight before we can safely prescribe your treatment.


If you’re transferring from another provider and requesting a higher dose, we also need to confirm your previous prescription.

We ask for:

  1. Photo ID

  2. Weight evidence (3 photos)

  3. Transfer evidence (if applicable)

Have everything ready before you start — all required photos must be submitted together. If we receive an incomplete set, we'll need to place your order on hold until the missing photos are provided.

If you need to come back later, you can exit and return when you're ready.

FAQs

Is my information secure?

Yes. All photos and documents you submit are encrypted, stored securely, and handled in line with UK data protection laws. We only collect the minimum information needed to safely prescribe your treatment. Your photo ID is deleted after 90 days.

What do you do with my photos, and when are they deleted?

Your photos are used only to verify your identity, confirm your weight, and review any previous prescriptions as part of your clinical assessment. They are seen only by licensed clinicians and are never shared with third parties or used for marketing.

Why might I be asked to upload evidence again?

Sometimes we need new photos even if you've already submitted them. This usually means something wasn't visible or didn't meet clinical requirements.

Common reasons include:

  • Only some of the required photos were submitted

  • The photo was blurry, dark, or cropped

  • Important details weren’t visible (e.g. weight reading, date, name)

  • The wrong type of evidence was uploaded

  • The image appeared filtered, altered, or edited

We understand this can be frustrating — clearer photos help us approve your order without delay.

Do I need to resubmit all photos if I receive an on-hold email?

No. If we've put your order on hold, you only need to resubmit the specific photo(s) mentioned in the email — not all four again.

What if I haven't finished and need to come back?

We need all your photos at the same time. If you submit incomplete evidence, your order will be placed on hold until we receive everything. Make sure you have everything ready before you begin.

What if I don't want to provide my photos?

Submitting photos is a clinical requirement — we're unable to prescribe your treatment without them. If you'd prefer not to submit your photos, you'll need to cancel your order. Your order will NOT cancel automatically, so please contact us at [email protected] to request a cancellation.


Detailed guidance

1. Photo ID

Examples of acceptable photo ID: a UK driving licence and a UK passport, each showing a clear photo, full name, date of birth, and identifying details.

We need one clear photo of a valid ID document to confirm your identity.

✅ Accepted ID types

  • Passport (any country)

  • UK or EEA driving licence (full or provisional)

  • Biometric Residence Permit (BRP)

  • National ID card (EEA, Norway, Iceland, Liechtenstein, Switzerland)

  • PASS card

  • HM Armed Forces or Police ID

  • Government-issued photocard travel pass

❌ Not accepted

  • Screenshots or photocopies

  • Expired documents

  • Work badges or student cards

✅ Your ID photo MUST:

  • Show the full document

  • Include all text, dates, and photos clearly

  • Be well lit (no glare, shadow, or blur)

  • Be a real, unedited photo of the physical document

2. Weight evidence

Examples of required weight evidence: a full-body front-facing photo in lightweight clothing, a full-body side-profile photo, and a photo of a person standing on scales with the digital weight clearly visible.

We need three separate photos to confirm your weight:

  1. Front-facing full-body photo

  2. Side-on full-body photo

  3. Photo of your current weight reading

These help our clinicians make sure the treatment is safe and appropriate for you.

Your body photos SHOULD:

  • Be taken today

  • Be clear and well lit

  • Show your full body and face

  • Be taken in lightweight, everyday clothing

  • Only include you

Your body photos should NOT:

  • Be filtered, edited, cropped, or altered

  • Be taken from old holiday or social photos

  • Hide or obscure any part of your body or the scale reading

  • Include other people

💡 Tip: Use your camera's selfie mode with the self-timer. Place your phone somewhere stable so you can step back and get your full body in frame.

Weight reading photo::

  • Both feet should be visible on the scales

  • The weight reading must be clearly visible

  • Avoid glare or reflections that obscure the display


3. Previous prescription evidence

(Only required if you're switching from another provider and requesting a higher dose than the starting dose.)

Examples of acceptable transfer evidence: a photo of a Mounjaro medication box with a clear dispensing label showing the patient’s name, date, and dose, and a screenshot of a previous order confirmation email showing the provider name, patient name, order date, and dosage.

To safely continue your treatment, we need to confirm your previous prescription.

You only need to upload one of the following:

Option 1: Photo of the dispensing label on your medication box

✅ MUST clearly show:

  • Your full name

  • Date prescribed

  • Medication name AND dose (e.g. 10 mg Mounjaro)

Option 2: Screenshot of your previous order or prescription

✅ MUST clearly show:

  • Your full name

  • Provider name

  • Date of previous order / prescription

  • Medication name AND dose (e.g. 10 mg Mounjaro)

If your screenshot is long or split across sections, you may upload multiple images.
