Uploading your supporting photos and evidence for weight loss treatment

What we need and how to upload it

For all new Phlo Clinic weight loss patients, we need to verify your weight before we can safely prescribe your treatment.


If you’re transferring from another provider and requesting a higher dose, we may also need to confirm your previous prescription.

We ask for:

  1. Weight evidence (3 x photos)

  2. Transfer evidence (if relevant)

Why do we ask?

These checks are required by UK clinical guidance and help us make sure your treatment is suitable, safe, and prescribed correctly.

FAQs

Is my information secure?

Yes. All photos and documents you upload are encrypted, stored securely, and handled in line with UK data protection laws. We only collect the minimum information needed to safely prescribe your treatment.

What do you do with my photos, and when are they deleted?

Your photos are used only to verify your identity, confirm your weight, and review any previous prescriptions as part of your clinical assessment. They are reviewed solely by licensed clinicians and are never shared with third parties or used for marketing.

Why might I be asked to upload evidence again?

Sometimes we need you to upload new photos even if you’ve already submitted them.


This usually means something important wasn’t visible or didn’t meet clinical requirements.

Common reasons include:

  • Only some of the required photos were submitted

  • The photo was blurry, dark, or cropped

  • Important details weren’t visible (e.g. weight reading, date, name)

  • The wrong type of evidence was uploaded

  • The image appeared filtered, altered, or edited

We understand this can be frustrating — clearer photos help us approve your order without delay.

Detailed guidance

1. Weight evidence

Examples of required weight evidence: a full-body front-facing photo in lightweight clothing, a full-body side-profile photo, and a photo of a person standing on scales with the digital weight clearly visible.

We need three separate photos to confirm your weight:

  1. Front-facing full-body photo

  2. Side-on full-body photo

  3. Photo of your current weight reading

These help our clinicians make sure the treatment is safe and appropriate for you.

Your photos SHOULD:

  • Be taken today

  • Be clear and well lit

  • Show your full body and face

  • Be taken in lightweight, everyday clothing

  • Only include you

Your photos should NOT:

  • Be filtered, edited, cropped, or altered

  • Be taken from old holiday or social photos

  • Hide or obscure any part of your body or the scale reading

  • Include other people

Weight reading photo tips:

  • Stand on the scale facing forward

  • Make sure your feet and the number are fully visible

  • Avoid glare or reflections

  • Smart scale screenshots are fine if the weight is clearly displayed


2. Transfer evidence

(Only required if you're switching from another provider and requesting a higher dose than the starting dose.)

Examples of acceptable transfer evidence: a photo of a Mounjaro medication box with a clear dispensing label showing the patient’s name, date, and dose, and a screenshot of a previous order confirmation email showing the provider name, patient name, order date, and dosage.

To safely continue your treatment, we need to confirm your previous prescription.

You only need to upload one of the following:

Option A: Photo of the dispensing label on your medication box

✅ MUST clearly show:

  • Your full name

  • Date prescribed

  • Medication name AND dose (e.g. 10 mg Mounjaro)

Option B: Screenshot of your previous order or prescription

✅ MUST clearly show:

  • Your full name

  • Provider name

  • Date of previous order / prescription

  • Medication name AND dose (e.g. 10 mg Mounjaro)

If your screenshot is long or split across sections, you may upload multiple images.
